Clinical Studies on the Conservative Treatment of Low Back Pain




Abstract
Low back pain has been called lumbago for a long time, a vague term meaning pain in the back and including a variety of condition.
It is common today to think that the low back and sciatica syndrome is a disease of civilization, a disease of poor physical condition, a disease of the automobile age.
The causes of low back pain has been considered in numerous reports and remains controversial in the field of Orthopedic Surgery.
Since the discovery of Mixter and Barr in 1934 that sciatica could be caused by a disc protrusion, it is general for a herniated lumbar disc to be the most common cause of low back pain.
In clinical studies of 410 consecutive series of low back pain which were treated conservatively from Sept. 1969 to Aug. 1974 in Severance Hospital, the following results were obtained:
1. The most common cause was herniated lumbar disc(59%), while the remaining causes included degenerative osteoarthritis of the lumbar spine(15.8%), trauma(11.9%), spondylolisthesis (5.4%), and etc.
2. No definite sex difference was present.
3. Above a half of all cases (51.7%) were included between 21 and 40 years of age.
4. The duration of illness was within 6 months in 64.9%.
5. Average hospital stay was 15.3 days.
6. The results of conservative treatment for low back pain were as follows: 146 (35.6%) were in excellent, 151(39.3%) in good, 54(13.2%) in fair, and 49(11.9%) in poor. Clinically satisfactory results could be obtained in 307 cases of them (74.9%).
7. The relation between duration of low back pain and result of conservative treatment was apparent. The shorter the duration, the better the result expected.
